The following up-to-date documents should be included in the packet, whenever possible:
Please Note: We’d rather have an old document with your hand-written/verbal updates than none at all.
Please ensure that client contact telephone numbers and email addresses, etc. are current!
- Cover Sheet describing client and current diagnosis, etc.
- Current Program Plan describing what your agency is doing
- CDER or similar document reflecting the clients functioning leveland…
- Recent Progress Review (if it provides additional info to the Current Program Plan)
- Current or former IL service plan (if available)
- Documentation/Notification of significant behavioral issues (when applicable)
- Current written plan addressing behavioral issues (when applicable)
- Psychological evaluation (if it accurately reflects your client’s current status)
- Social evaluation (if it accurately reflects your client’s current status)
- School records (if applicable, as in clients newly graduated from school)
- Anything else you think will help us understand your client better!

After we receive the referral, we will contact you to let you know we received it.
We will contact the client and set up a time to meet, then let you know.
If we have any problems getting in touch with the client, we’ll let you know!
We encourage clients to invite anyone they want to the meeting.
You do not have to attend the meeting, but if you want to, please let us know at the time of your referral.
At the meeting we will conduct an informal assessment which includes discussing the client’s need for services, motivation level, skill areas and abilities, experiences in other programs, etc.
After the meeting, we will call or email you to let you know how the meeting went and to discuss anything that came up that might require additional discussion. At that time, we should have a good idea of how we can best serve your client.
You will receive a Service Request indicating the service skill areas and number of hours.
OCIL will not begin services until we have a purchase of service (POS) in place indicating start date.
